Please return by postage ( 151 Darley Rd , Manly , NSW , 2095 , Australia ) the original damaged testamur where applicable and provide a signed statutory declaration outlining the circumstances and events leading to the damage.
In instances where the testamur cannot be returned, a Statutory Declaration must be completed and signed by a Justice of the Peace.
Please note that your replacement testamur will include the word "COPY" at the bottom of the certificate.
